A worker at a McWane Inc. facility in New York was killed in a workplace accident on December 14, according to sources. Michael Donahue, 50, died while making repairs to a hydraulic tabletop in the mold department of a factory. Officials from the US Department of Labor’s Occupational Safety and Health Administration (OSHA) have notified Kenedy Valve—McWane’s parent company—of an investigation into the incident.
Donahue worked at the facility in Elmira for two years. Firefighters were called to the facility around 9:30 p.m. on the Friday night, after receiving calls for assistance. The accident remains under investigation, although Elmira City Police Chief Michael Robertson told reporters the initial investigation ruled the cause of death to be accidental.
“In general terms, we will review the scene, conduct interviews, take photos and measurements and review OSHA-required hazard and safety programs,” said OSHA’s Christopher Adams. “We are going to review what happened to see if there were any OSHA violations.”
