An employee of the Grand Rock Co. Inc. lost four fingers to an unguarded machine responsible for bending tubes in June. The U.S. Department of Occupational Safety and Health Administration (OSHA) inspected the Painesville, Ohio-based company and will fine the company for $52,500.
“It’s unacceptable that Grand Rock Co. failed to provide adequate machine guarding in its facility and that a week after this tragic incident, the machine remained unguarded and in use, exposing other workers to similar injuries,” said OSHA’s Cleveland director. “Employers must take all precautions to protect workers from known hazards that exist in their facilities.”
OSHA inspectors found that Grand Rock Co. willfully failed to guard the machine responsible for the finger amputation. A willful violation is that which is perpetrated with full knowledge of the legal safety standards. Inspectors also identified a serious violation, one that could result in death or serious injury, for lacking a lockout/tagout protocol.
