The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) inspected a San Cast Inc. facility in Coshocton, Ohio after a worker’s leg was amputated in June 2013. Even after the tragic accident, OSHA discovered multiple amputation and fall hazards during a follow up inspection in November.
OSHA issued 17 citations that amounted to more than $150,000 in fines. Several citations were for repeat violations, which carry stricter penalties and higher fines.
San Cast received repeat violations for inadequate fall protection stemming from an unguarded platform. Management also failed to protect workers from ingoing nip points on belts and pulleys, despite a previous OSHA citation.
