The US Department of Labor’s Occupational Safety and Health Administration (OSHA) has cited a facility in North Jackson, Ohio, with 8 safety violations, according to WKYC-TV. OSHA officials inspected the Extrudex Aluminum facility following a fatal accident a little over a year ago.
John Tomlin Jr., 21, died last year when he was crushed to death underneath a cart of hot aluminum parts.
OSHA officials determined that management acted with disregard or indifference to safety regulations that resulted in the accident. The report alleges that Extrudex Aluminim “could have prevented this tragic loss by protecting workers from hazards unique to its operation.”
“Workers should not be asked to take such risks, and OSHA will not tolerate such disregard for human safety,” said OSHA administrator Howard Eberts.
This inspection resulted in a $175,000 fine and places Extrudex in OSHA’s severe violators program. It will be subject to more OSHA inspections in the future.
