Mississippi Phosphate Corp. received 40 safety and health violations from the US Department of Labor’s Occupational Safety and Health Administration (OSHA) following an inspection, according to CBS News. OSHA officials inspected the facility in Pascagoula, Mississippi following the death of two workers in separate incidents.
On May 22, a worker at the facility was killed while attempting to start a steam turbine when he was hit by metal debris. On June 1, a worker starting another steam turbine was also killed by flying debris. In this second incident, the turbine was turned over on its side when the worker attempted to restart it.
OSHA issued citations for exposing workers to hazards and failing to protect them against over pressurization. OSHA also determined that Mississippi Phosphate failed to inspect and test equipment. Proposed fines total $165,900.
“The company is undertaking a comprehensive review of OSHA’s report and will expeditiously address OSHA’s recommendations for improving the safety of our workplace,” said company vice president Richard Johnson. “MPC remains committed to providing a safe and healthy workplace for its employees.”
